In addition to signing up for a Winter sport and completing and submitting the Interval Health History Form, every Student-Athlete must have a current Health Examination (physical) on file in the health office BEFORE the start of each season. A Health Examination conducted on or after November 1, 2024, is valid for participation in a Winter Sport. If you have already submitted a copy of your current health examination(physical) to the nurse, you do not need to submit another copy.
